Exemestane is an oral steroidal aromatase inhibitor used primarily in the treatment of hormone receptor-positive breast cancer in postmenopausal women. It works by irreversibly inhibiting the aromatase enzyme, which is responsible for the conversion of androgens to estrogens in peripheral tissues.
By blocking estrogen synthesis, exemestane helps reduce the stimulation of estrogen-sensitive breast tumors, thereby slowing or preventing their growth. It is typically prescribed as an adjuvant therapy following initial treatment with tamoxifen or as a first-line treatment option.
Exemestane is contraindicated in:
- Premenopausal women
- Patients with known hypersensitivity to exemestane or any of its components
- Patients with severe hepatic impairment
Caution is advised in patients with a history of cardiovascular disease, osteoporosis, or other conditions that may be affected by changes in estrogen levels.
The recommended dosage of exemestane is 25 mg once daily, taken with or without food. Treatment is usually continued for 2-5 years, depending on the individual patient’s response and tolerance.
